Optional Life Benefit Summary:
Employees pay 100% of the following benefits.
Employee Optional Life Insurance Benefit
Employee Optional life is available in $10,000 increments to a maximum of
$500,000.
Any amount applied for that is less than $250,000 will be guaranteed with no
underwriting or proof of good health. You must submit a Statement of Health
Form form for any amount of optional life insurance over the guaranteed issue
amount (GI).
Spouse Optional Life Insurance Benefit
Spouse Optional Life Insurance is available if the employee buys optional life
insurance on themselves. Spouse Optional Life is available in $5,000 increments
to maximum of 50% of the employee's amount. A Statement of Health Form must be
completed for ALL Spouse Optional Life Insurance applied for.
Child(ren) Optional Life Insurance Benefit
Optional Life Insurance on your child or children is available if the employee
buys optional life insurance on themselves. Child(ren) Optional Life is
available for $5,000 or $10,000 to maximum of 50% of the employee's amount. A
Statement of Health Form is NOT required for Child(ren) Life Insurance.
