Long Term Disability Benefit Summary
QVine Corporation pays 100% of the following benefits for all active,
full-time employees working at least 30 hours per week.
Long Term Disability Benefits (LTD)

Long Term Disability benefits will pay 60% of the employee's monthly salary up
to a maximum monthly benefit of $6,000.

Benefits will be paid for the duration of the disability. Benefit reductions
will occur at age 65.

In order to receive benefits, the Long Term Disability Claim Form that can be
found on this website must be completed by you, your doctor and your employer.

Long Term Disability benefits are paid on a monthly basis.

Checks will be sent to your home address.

90 day elimination period before Long Term Disability benefits begin.

Your benefit will be reduced by amounts you receive from Social Security, an
employer sponsored retirement plan and other group disability benefits.

A pre-existing condition is a condition for which you received medical
treatment, consultation, care or services including diagnostic measures, or
took prescribed drugs or medicines in the 3 months just prior to your effective
date. Pre-existing conditions are excluded from coverage if a disability due to
that condition begins in the first 12 months after your effective date of
coverage.

Disabilities primarily based on self-reported symptoms or disabilities due to
mental illness have a limited pay period of up to 24 months unless you remain
hospital confined at the end of the 24-month period.

Claim forms should be filled out by the employee and submitted to Human
Resources to be included with the employer portion of the claim information.

Can I return to work part-time and still receive a benefit?
Yes. The LTD plan provides financial incentives for you to return to work, even
on a part-time basis. For the first 24 months of disability benefits, you may
receive up to 100% of your pre-disability earnings when combining
Rehabilitation Incentives, Family Care Expense reimbursement, and part-time
earnings.
Does the plan have limitations?
The LTD plan does have limitations. For example the plan does not cover
pre-existing conditions, unless your disability begins after you have been
covered under the plan for 12 months. A pre-existing condition is an injury,
sickness, or pregnancy for which you in the 3 months before your effective
date: received medical treatment, consultation, care, or services; took
prescription medications or had medication prescribed; or had symptoms or
conditions which would cause a reasonably prudent person to seek diagnosis,
care, or treatment.
The plan also has limited benefits for particular conditions. Monthly benefits
are limited to 24 months during your lifetime if you are disabled due to Mental
or Nervous Disorders or Disease, Neuromusculoskeletal and Soft Tissue
Disorders, Chronic Fatigue Syndrome or Alcohol, Drug and Substance Abuse or
Dependency.
Exclusions
This plan does not cover any Disability which results from or is caused by or
contributed to by: war, insurrection, or rebellion; active participation in a
riot; intentionally self-inflicted injuries or attempted suicide; or commission
of a felony.
