Eligibility

Employee Eligibility:
All full-time QVine Corporation employees are eligible for benefits on the first of the month following their date of hire.

Dependent Eligibility:
Children are covered through the end of the month in which they turn 26.

Changes in Enrollment Status:
Open enrollment will occur once a year. Each year you may change your coverage level, if desired, with no underwriting or pre-existing conditions. Once you have made your selection, you may not change it until the next year's open enrollment unless you have a "life changing event." Life changing events include:

• Marriage
• Divorce
• Birth or adoption of a child
• Death
• Spouse losing insurance coverage due to change in job

In the event that you have a "life changing event", you must notify Human Resources within 30 days if you wish to change the status of your insurance coverage.
